Update: Blue Cross Medicare Advantage Annual Health Assessment (AHA) Incentive

Several of your patients haven’t completed an annual health assessment (AHA). We encourage you to set up an in-person or telehealth AHA with our Blue Cross Medicare Advantage members who haven’t completed an AHA.

• Step 1 - Get Your Target List: Send an email to our team with your full name and NPI to request access to Indices. Indices allows you to identify which Blue Cross Medicare Advantage (PPO)^SM and Blue Cross Medicare Advantage (HMO)^SM members haven’t completed an AHA.


• Step 2 - $300 incentive: We encourage you to leverage your target list to set up an AHA appointment with our Blue Cross Medicare Advantage members. For every eligible AHA you complete between Jan. 1, 2021 through Dec. 31, 2021, you'll receive an incentive payment of $300 per Blue Cross Medicare Advantage (PPO) and Blue Cross Medicare Advantage (HMO) member, in addition to your contracted rate.*
